In a penalty shootout when one team fields less players than the other (sent off or injured) and you reach the point where every player on the pitch should, in theory, take his penalty, the team that has numerical advantage in real life gets to skip a player (it will obviously choose a player that feels uncomfortable taking a penalty), so that there is an even number of penalty takers (e.g. 20 - 10 per side).
The match engine does not know that. I just lost an African Confederation Cup final because my player Nr. 11, my goalie, missed - while Al Ahly, who were down to 10, had their No 1 penalty taker Ricky Centurión taking already his second penalty. So basically I got punished for not having a weak penalty taker sent off.
This is a bug that has persisted for various years now.
