This is an issue with clubsthat have "II" teams, particularly in the Bundesliga. For example, Stuttgart has a II team. If you take any 17 year old player from the U19 who has never played in the first or II team and play him in a senior match, he will not receive a milestone for 1st senior appearance and there will be no story.
However, clubs without a II team, such as RB Leipzig, will get this milestone.
