How are you setting up defending set pieces and corners? Are your tall players, best headers and jumping reach all back to? Also I like to put my smaller players who have high bravery, anticipation to either stay back or rush out ready to get the ball as its comes out of the defence. Do you man mark their best headers, tall players etc? If you do all that then maybe your players just aren't good enough but I suspect if its happening in most games it is down to how you train and prepare for game.
