I am not sure how it's happened, never seen this in all my years of playing FM. I ended up just approaching his team after the loan ended and negotiated the price, although you can see in the screenshot that it was already accepted, strange one!
I know it'll probably be hard to re-produce for your own investigative matters, although the steps below were;
Player had 2 weeks left on loan, I wanted to extend it to next season so I asked - this was accepted
I negotiated an extension of the loan and they included a future mandatory fee after this extension ended - I agreed
Contract negotiations then instantly come up with the player - he agrees
I welcome him to the squad and get a player to introduce himself and assign a squad number
Signed a player although he's back at his original club
in Transfers, Contracts, Scouting, Recruitment Meetings and Staff Responsibilities
