This is still happening. Pepe of Arsenal has converted just 3 of 8 penalties now (16Pen 14Fin 15Tec 10Dec (admittedly bad decisions rating).
What's worse is that the player rating tanks after missing a penalty, irl a player wouldn't just play crap after missing a penalty, that's kinda the point in sports psychologists... I either need to see out the rest of the game with someone with a =/<5.0 rating or sub them off.
If this was User/AI equal, surely the opposing GK rating should go to =/>8.0 after saving a Pen? It seems to penalise my player a lot.

Apologies for getting to this one so late. I was having the same issue when promoting/relegating teams from Prem/Championship - In the end I tested each league seperately and they all came back verified... Maybe do the same? I think I changed a couple settings here and there, but I don't think it was anything important in the end nor do I believe those changes effected the issue I was having.
