Description of issue
First issue I believe my goalkeeper got injured in the same highlight as my team giving away a penalty, so the penalty was taken with no goalkeeper in net (West Ham v Arsenal, 71st minute)
Second one I think my goalkeeper got injured just after I made all 5 subs and the game continued as normal without enforcing a stop in play so I could put someone in goal, watching the highlights back in comprehensive it seems the game did automatically put a player in goal but that didn't show in the tactics screen and it looked like I was playing with 10 men and no one in goal (Leeds v Arsenal, 77th minute)
