Cup eliminations not working

Okay, so I'm at a loss. 

 

I'll layout what exactly I'm looking to do. 
I've three leagues tiers: A, B and C.
A: 16 teams
B: 18 teams
C: 20 teams

I want three rounds to go into a group stage and I've laid it out as following

 

1st Round
Teams added: 12
Matches: 6

 

2nd Round
Teams added: 26
Matches: 16

 

3rd Round
Teams added: 16
Matches: 16

 

Basically, I want the 1st round to draw 12 teams from League C; I want the 2nd round to draw 8 teams from League C and 18 teams from League B; and I want round 3 to draw 16 teams from League A 

The issue I'm having is that the teams eliminated from the 1st round are being reentered into the 2nd round; I'll post the 1st, 2nd and 3rd rounds from in game and you'll see the issue I'm getting. 

 

 

I've no idea what's going on.

You have a ranking problem in the Basic Rules. Finish your cup, test if it runs OK-ish in the game and then convert to Advanced Rules.

The error is in the software, not in your file btw.
